Is it better to use the original formula or the combined formula? Master Ni Xia Hai told us the answer with examples.

In recent years, with the tide of TCM classics, people began to attach importance to classics again, and the clinical application of classics increased, which is a good thing. However, there have always been two kinds of voices about the use of classics. One thinks that Zhang Zhongjing's original quantity should be used as much as possible, and it is not necessary to add or subtract it at will, let alone use it together at will. Another view is that the classic prescriptions should also be "added and subtracted according to the symptoms", which can not only change the dosage, but also increase or decrease the drugs and can be used in combination. As a beginner of classic prescriptions, you will face a dilemma. Is it better to use the original prescription or add or subtract with the symptoms? Which of the above two views is more correct?

I was puzzled for a long time when I first started to learn classic prescriptions. After studying the course of master Ni, he gradually understood how to use the classic prescription. Today, the author will tell you the correct use of the classic prescription through a real medical case of Ni!

Patient HN, female, 54 years old, was admitted to the hospital on April 2, 20081,and she was overweight.

Reasons for seeing a doctor: heavy body heat (the patient is in menopause), fat body, want to lose weight in traditional Chinese medicine, a little pain in hands and joints, pain in feet, fatigue, depression and a little cough. These are the main symptoms of patients and the reasons for seeking medical treatment.

Consultation: Daily defecation, excessive sleep and lethargy. Have a good appetite I'm thirsty and like to drink at room temperature.

Pulse diagnosis: slight and weak.

Tongue diagnosis: pale and dry.

Analysis: The patient complained of heavy body heat, thirst, like to drink at room temperature and have a good appetite. We first thought that this should be a heat syndrome, and Yangming must have heat. But if we stop there, the diagnosis will not be comprehensive enough, because the patient has other obvious symptoms that Yangming fever can't explain. For example, patients have drowsiness, feeling tired and weak, and their pulse is small and weak. These symptoms are obviously manifestations of yang deficiency. Friends who are familiar with Treatise on Febrile Diseases should be able to think of Shaoyin disease. In addition, the patient still has problems such as depression, hand joint and heel pain, which can not be explained by Yangming disease. It seems that the patient is a complicated illness, which should be mixed with cold and heat.

Readers, if you can think of this, it shows that your Chinese medicine foundation is not bad, at least in the dialectical general direction. So, how is Teacher Ni dialectical?

Ni's diagnosis is: upper heat and lower cold. If the classic prescription is used, which prescription should be used? Going back to the problem mentioned at the beginning, if it is the first view, we must find a prescription that can solve both the upper fever and the lower cold. Alas, some students had a flash of light. Isn't this syndrome of cold and heat "Jueyin disease"? How about ebony pills?

According to the syndrome analysis, Jueyin disease belongs to the syndrome of six meridians, with yin and yang dominating recovery and cold and heat mixed. Jueyin disease should be clear and warm. Wu Mei Pill is a representative prescription of Jueyin disease. Isn't this very consistent with the patient's condition? If we compare the outline of Jueyin disease: "Jueyin is a disease, quenching thirst, attacking the heart with qi, feeling hot in the heart, being hungry and unwilling to eat, and vomiting after eating, the benefits will not stop." The patient has no other symptoms except thirst. Obviously, Wu Mei pills are not the best prescription.

So, how did the classic master Ni open it?

Let's start with acupuncture. The patient has pain in hand joints and heels. Ni used acupuncture to relieve pain, Sanjian Touhouxi to treat hand joint pain, and Daling, Shuiquan and Fushen to treat heel pain. Here, I will focus on explaining Daling point. Why is Daling point used for heel pain? Because acupuncture has a symmetrical point selection method, Ni Shi said, if we press the palm down, is the position of Daling point symmetrical with the position of heel? That's right, that's why there is such a symmetrical acupoint selection method, and it's also right to say that it's a holographic acupoint selection method.

Then look at the classic prescription. Does Master Ni use classic prescriptions? Of course not! But the combination of the four classics! First, ephedra, aconite and asarum were used. Isn't this Mahuang Fuzi Asarum Decoction? It can solve shaoyin disease, that is, the problem of cold. Then use gypsum and Anemarrhena asphodeloides, white tiger soup, of course! Aiming at the problem that patients have fever or Yangming fever. Then Nishi used Pinellia ternata, Magnolia Officinalis and Perilla Leaves, which are the main drugs of Banxia Xiexin Decoction. It can be used for eliminating phlegm, relieving sore throat, solving climacteric problems, and treating depression. Don't you take medicine for joint pain and heel pain? Of course you have to use it! Mu Zhi Decoction of Guizhi and Shaoyao, Guizhi, Radix Paeoniae Alba, Atractylodis Rhizoma, Radix Saposhnikoviae, Herba Ephedrae, Radix Aconiti Lateralis Preparata, and Rhizoma Anemarrhenae are used for Ni's treatment. Isn't that Guizhi Shaoyao Mu Zhi Decoction? Finally, patients want to lose weight, how to reduce kidney deficiency! Therefore, Nishi used Radix Rehmanniae, Rhizoma Alismatis and Fructus Psoraleae to tonify the kidney. At this point, the prescription is complete! Let's take a closer look. Is Teacher Ni's prescription good?

If this prescription goes on, it will be strange if there is no curative effect! Isn't it? The medical record of classic prescription master Ni tells us the problems mentioned at the beginning very directly. Of course, the classics can be used together and should be used together. The key to the problem lies in the accuracy of dialectics. Then, when selecting prescriptions, we should first consider the classic prescriptions, including the prescriptions with certificates and the medicines with certificates. This is the real classic thinking and the correct use of classic prescriptions!
