At 8: 42: 40 in the morning, when the vehicle was driving to Xiabulong Road in the northbound direction of Longhua section of G94 Pearl River Delta Ring Road, the Guangdong B2 1288 tractor suddenly separated from the Guangdong BRB92 semi-trailer, causing the front end of the Guangdong BRB92 semi-trailer to touch the ground, damaging the oil tank, and the diesel oil in the tank leaked about 17.5 cubic meters. The accident caused direct economic losses of about 456,600 yuan, without causing casualties.

After investigation, driver Huang Mouhua did not operate properly when connecting Guangdong B2 1288 tractor with Guangdong BRB92 semi-trailer, and the inspection was not in place. No hidden danger of safety accident was found, which was the direct cause of the accident. At the same time, Nanhu Company did not improve the safety management rules and regulations and safety operation procedures, and the safety education management for workers was not in place, and the safety education records were incomplete; Huang Mouqiang, the main person in charge of the company, Yang Mouhua, the person in charge of transportation safety, and Sun Mouliang, the safety manager, failed to organize the formulation of safety operation procedures for the connection of tractors and semi-trailers, the formulation and implementation of the safety production education and training plan of the unit, and the investigation of potential safety hazards that may cause accidents due to the connection failure of tractors and semi-trailers.

The accident investigation team suggested that the emergency management department of Longhua District should impose administrative punishment on Nanhu Company according to the provisions of Item (3) and Item (4) of Article 94 of the Law of People's Republic of China (PRC) on Work Safety; According to Item (1) of Article 92 of the Law of People's Republic of China (PRC) on Work Safety, Huang Mouqiang, the principal person in charge of the company, and Yang Mouhua, the person in charge of transportation safety, were given administrative punishment. It is suggested that Nanhu Company handle its safety manager Sun Mouliang and tractor driver Huang Mouhua internally.
