What is fibromyalgia?

Fibromyalgia is a common group of non-joint rheumatic pain, which is characterized by muscle tendon attachment and adjacent soft tissue pain, tenderness and stiffness.

The term myalgia refers to muscle pain. Myositis is caused by inflammation of muscle tissue. The word myositis is not accurate enough for fibromyalgia which lacks inflammatory response. Fibromyalgia refers to the pain in fibrous tissues, muscles, tendons and ligaments. Any fibromuscular tissue can be involved, such as occipital region, neck (neck spasm), shoulder, chest (chest myalgia), lower back (lumbago) and thigh (thigh pain and quadriceps femoris).

Mild fibromyalgia can spontaneously subside with the release of tension, but it may often recur or become chronic. In order to reassure patients and show that the disease is benign, stretching exercise, aerobic exercise, improving sleep, local hot compress and gentle massage can all alleviate the disease. Taking a small dose of tricyclic antidepressants (such as amitriptyline 10mg or minimum effective tolerance dose) before going to bed can deepen sleep and regulate pain. Aspirin 650 mg/kloc-0 once every 3~4 hours, or other non-steroidal anti-inflammatory drugs in sufficient dosage, has no effect in clinical trials, but it may be helpful to some people. You can simply inject 1% lidocaine 1ml or 2ml at the tender and weak part, or inject it together with hydrocortisone acetate suspension 20~40mg (see soft tissue injection for chronic low back pain). If a drug has a drowsy effect, you can switch to other similar drugs (small doses). Taking serotonin specific inhibitors (such as florfenicol HCl 10mg or 20mg) in the morning can relieve depression and improve symptoms. Care must be taken to avoid aggravating sleep problems due to medication, otherwise it will cause insomnia. The functional prognosis of patients with comprehensive support therapy is good, although the symptoms of different degrees can last. For the treatment of anxiety or depression, more active special methods and patients' cooperation and support are needed. In a word, the best treatment should be individualized, comprehensive and adjustable, which requires patients' direct participation.
