Employees of catering enterprises are welcome to apply for health certificates in our hospital.

Health certificate application process

First, individuals apply for health certificates.

1. Make an appointment online or by phone. Specific contact information can be found in official website of Zhikang Physical Examination.

2. Have a light diet the day before the physical examination, do not drink or stay up late, so as not to affect the liver function.

3. Bring your ID card and check on an empty stomach on the day of physical examination. The physical examination starts at 8: 00 in the morning.

4. Pay at the front desk and print the guide.

5. Physical examination process: blood drawing-internal surgery-anal swab (bacterial culture) -DR chest film.

6. After the physical examination, return the checklist to the front desk.

7. You can get a health certificate from the physical examination center within 3 working days after the physical examination. If you need to mail it, you can collect it by SF Express.

Second, the enterprise group to handle the health certificate

1. Contact the health consultant to make an appointment for the time and method of physical examination.

2. Organize unified physical examination for employees. If there are many people, try to divide them into batches.

3. Inform employees of the matters needing attention in physical examination.

4. Inform employees to hand over the physical examination form to the recycling desk after the physical examination.

5. Wait for the health certificate for about 7 working days. If it is urgent, please ask the health consultant in advance.

How much is the health certificate?

The health certificate fee of the intellectual fitness center is 100 yuan/person. If training is needed, 1 10 yuan/person.
