Handling process of hygiene license for catering industry

First, the newly opened catering industry shall submit the following materials when applying for the Hygiene Permit:

1, health permit application;

2, the construction project design hygiene review approval;

3, the completion of the construction project health acceptance letter;

4, restaurant business premises layout (to indicate the kitchen function, warehouse, restaurant and other locations and their corresponding areas);

5. List of sanitary facilities;

6, water quality hygiene inspection report;

7, employees health examination and health training certificate;

8, health management organization and system;

9 other materials required by laws, regulations, rules and standards.

Two, catering units hygiene license is valid for two years, review once a year. The examination time shall be subject to the issuance time of the health permit, and shall be declared 30 days in advance each time, and the following materials shall be submitted:

1, health permit application;

2, the catering industry unit name, legal representative, business projects, layout, facilities and other commitments consistent with the original approval;

3, employees in the new year health examination and health training certificate;

4, water quality hygiene inspection report;

5, tableware, food hygiene inspection report within six months;

6, the original health permit (original and copy);

7. Other materials required by laws, regulations, rules and standards.

Three, catering units need to continue to apply for a health permit (replacement), should apply thirty days before the expiration of the health permit, and submit the application materials according to the following requirements:

1, health permit application;

2, the catering industry unit name, legal representative, business projects, layout, facilities and other commitments consistent with the original approval;

3, employees health examination and health training certificate;

4, water quality hygiene inspection report;

5, tableware, food hygiene inspection report within six months;

6, a copy of the original health permit (original and copy);

7. Other materials required by laws, regulations, rules and standards.

Procedures for handling hygiene license of catering industry

1. All application materials are in duplicate, arranged in sequence, printed on A4 paper and stamped with the official seal of the applicant item by item.

2. If there are errors in the application materials that can be corrected on the spot, the applicant is allowed to correct them on the spot.

3. If the application materials are incomplete or do not conform to the statutory form, the applicant shall be informed of all the contents that need to be supplemented at once on the spot or within five working days. If the applicant meets the requirements after supplementing the materials, it shall be accepted. Fails to inform, as of the date of receipt of the materials.

4, the administrative department of health from the date of acceptance within twenty working days to make a decision of approval or disapproval. If a decision cannot be made within 20 working days, 10 working days may be extended upon approval, but the applicant shall be informed of the reasons for the extension. Where there are other provisions in laws and regulations, if inspection, testing, quarantine, appraisal and expert review are required according to law, the time required shall not be counted in the period of health administrative license.
