What are the symptoms of lumbar tuberculosis?

(1) Lumbar mobility is limited. The disappearance of physiological curvature of the waist and the protective spasm of paraspinal muscles make the waist in a highly protected state, which limits the activity of the spine and relieves pain. This is one of the obvious signs in the early stage of lumbar tuberculosis. When picking up things from the ground, bend your hips and knees and squat straight as much as possible to avoid bending down to pick up things, that is, the pick-up test is positive. Prone spine extension test was also positive.

(2) Spinal deformity. When the lesion develops to a certain extent, due to the collapse and deformation of the vertebral body, lumbar kyphosis is obvious and scoliosis is rare. Protrusion is often accompanied by obvious tenderness and percussion pain.

(3) Cold abscess. Cold abscess is of great significance to the diagnosis of lumbar tuberculosis. Because the abscess forms slowly and has no acute inflammatory characteristics, it is called "cold abscess". Purulent fluid is generally thin and rice soup-like, and contains a lot of tuberculous granulation tissue, cheese-like substances, necrotic intervertebral disc tissue and dead bones. Cold abscess of lumbar tuberculosis is mostly located on one side or both sides of psoas major muscle, which can cause irritation symptoms of psoas major muscle and lead to flexion contracture deformity of hip joint, and Thomas sign is positive. It can also be injected into bilateral iliac fossa, lumbar triangle and femoral triangle along psoas major muscle, and a lasting sinus is formed after the abscess ruptures. If there is secondary infection, it is accompanied by acute suppurative inflammation.

(4) Systemic symptoms. Early patients may have no symptoms, but also chronic consumptive manifestations such as general malaise, fatigue, low fever in the afternoon, loss of appetite and emaciation.

Laboratory examination: patients with pulmonary tuberculosis have low hemoglobin and low total white blood cells, but high lymphocytes and rapid ESR.

(5) the x-ray situation. Osteoporosis, narrowing of intervertebral space and blurring of upper and lower edges of vertebral body appeared in the early diseased vertebral body. In the late stage, the intervertebral space disappeared, the vertebral body was destroyed, dead bones and cavities were formed, and the shadow of psoas major was widened.
